Using the OFQC within the university package

The Occupational Formulation Quality Checklist (OFQC) was developed to support occupational therapists to review the quality of their MOHO-based occupational formulations.  It was developed with reference to the guidebook by Parkinson and Brooks (2021), the Model of Human Occupation, and OccupationalFormulation.​com material, and has been refined following feedback from occupational therapists and educators. User feedback is welcomed to support further refinement of the checklist and can be provided using this form.

The OFQC is made available to university course instructors for the following purposes:

  • To support instructors in developing scenario-based occupational formulations

  • To support instructors’ assessment of students’ occupational formulations

  • For students to use in class to review occupational formulations

Please do not provide copies for student use outside of class.
